Please complete your personal details on the National Grades Application form for Yondan or Godan.  Download the form from the J.F.A. web site via the link below:

Fill in details on the Yondan/Godan Application Form, together with the acknowledgment of all prerequisites for the grade you are applying for on the back of this notice.

Attach this Acknowledgement of Prerequisite Form to your Completed Application Form.

The Judo SA Administrator will confirm that you are a Member of Judo SA at the time of your Application in the space below.

Make sure you also send your non-refundable $50 Admin Fee either by cheque, money order or direct to Judo SA, or by EFT: BSB 105 900 Acc No. 954110440.

Note: Please Identify the grade payment with your name.
